    These procedures are based on the following stipulation: "The chief administrator (e.g., department head) stipulated in Article 1, Section 14 of Chemical Control Rules must assign storage cabinet administrators for all the storage cabinets under his/her control. The workplace administrator, however, may double as the storage cabinet administrator."

    Completion of Forms 6 and 7: Use Record of Rank 3 Chemicals

1) Poisons, etc. (Complete a form for each container.)
  • User name : Enter the name of the person who brings in, produces or repackages the chemical into small sizes.
  • Storage place : Enter the building name and room name, such as Sample Preparation Room ##IN, Storage Ring.
  • Storage Cabinet No. : Number the storage cabinets so that you can distinguish them.
  • Chemical name : Enter the trade name and the common name or substance name.
  • Class : Circle the applicable item.
  • Container No. : Indicate the specific numbers designated to individual containers so that they may be distinguishable.
  • Net content and Gross weight : Enter the value including the unit such as 100g and 500ml.
  • Date of bringing in : Enter the date of registration with the storage cabinet.
  • Objective : Enter the objective of use such as measurement and experiment.
  • Enter its reasons in the Objective column when you have used up, disposed or moved/relegated them.

2) Deleterious substances (Complete a form for each chemical.)
  • Chemical name : Enter the trade name and the common name or substance name.
  • Class: Circle the applicable item.
  • Storage place : Enter the building name and room name, such as Sample Preparation Room ##IN, Storage Ring.
  • Storage Cabinet No. : Number the storage cabinets so that you can distinguish them.
  • Date of obtainment : Enter the date when the deleterious substance is brought in, produced or repackaged into small sizes.
  • User name : Enter the name of the person who brings in, produces or repackages the deleterious substance into small sizes.
  • Container No. : Indicate the specific numbers designated to individual containers so that they may be distinguishable.
  • Package volume : Enter the net content or weight of the product when it is repackaged.
  • Disposal date : Enter the date of disposal of a container. If a container with a certain amount of the chemical remaining is disposed of, moved or transferred, indicate that accordingly.
  • Disposing person : Enter the name of the person who has taken the procedure to the Facility Administration Department.


4) Inventory count of chemicals

    This refers to the stock status report for rank 3 chemicals stipulated in Section 6 (3) of Detailed Rules on Chemical Control Rules (duty of the workplace administrator). Please report to the chief administrator (e.g., department manager) biannually (at the end of September and March) by the end of the month following the month of inventory.

  2. Inspection before the start of use
    The draft chamber must be inspected for the following items when you use it for the first time or when you modify/repair it.
    1. State of dust deposits in duct and exhaust fan
    2. Presence or absence of slackness of duct connection
    3. Air intake and exhaust capacity
    4. Requirements for maintaining equipment performance in addition to above-mentioned three items
  1. Regular voluntary inspection
    Voluntarily inspection of the following items must be performed regularly at least once a year.
    1. Presence or absence and severity of abrasion, corrosion, indentation, and other damage to hood, duct and fan
    2. State of dust deposits in duct and exhaust fan
    3. Lubrication status of exhaust fan
    4. Presence or absence of slackness of duct connection
    5. Operating condition of belt connecting electric machinery and fan

    7. Requirements for maintaining equipment performance in addition to above-mentioned three items
